Cross-Functional Team Assessment Registration Form

Thanks for your interest in the Cross-Functional Team Assessment! This tool will help team members assess their: 

• Level of trust/partnerships
• Degree to which they feel their goals are aligned and roles are clear 
• Decision-making practices
• Ability to collaborate and influence
• Ability to keep each other informed
• Meeting practices

A couple key things to know about this assessment:

• Since this is a pilot program, we would offer it to you at cost.
• Total time investment would be 25-35 minutes for raters to complete, 1 hour for us to review results with you, and 1 hour for the full team to review and discuss.
• The assessment process would take 3-4 weeks from start to finish; we'd love to have the pilot complete by end of May if possible but otherwise we are flexible.
